Pharmacological treatment of psychotic depression : in search for evidence

The main focus of this thesis was to establish the best pharmacological treatment for unipolar psychotic depression.
In a Cochrane meta-analysis we concluded that treatment with an antidepressant and an antipsychotic has not been proven more effective than an antidepressant alone, but more effective than an antipsychotic alone. Nevertheless, guidelines often recommend treatment with the combination. Thus, guidelines are not always evidence-based.
In a randomized double-blind multi-center trial (RCT) in patients with a psychotic depression (N = 122) we compared the antidepressants imipramine and venlafaxine with a combination of an antidepressant and an antipsychotic (venlafaxine plus quetiapine). At the primary outcome measure (response) venlafaxine plus quetiapine was more effective than venlafaxine with no significant differences between venlafaxine plus quetiapine and imipramine en between imipramine and venlafaxine. The combination was well tolerated, although showing more weight gain.
In an open study the responders in the RCT were followed for 4 months. Continuation treatment with the same medication to which the patients had responded, remained effective and most patients further improved. The non-responders in the RCT were treated with lithium-addition for 4 weeks. Nine of 15 patients became responder.
Conclusion: By importing the data from this RCT in the meta-analysis, the difference between the combination and antidepressant monotherapy becomes statistically significant. So, for the first time there is evidence that a psychotic depression should be treated with the combination of an antidepressant and an antipsychotic (venlafaxine plus quetiapine) and not with an antidepressant alone (venlafaxine).

De farmacologische behandeling van psychotische depressie: op zoek naar bewijs.

In dit proefschrift is onderzocht hoe de unipolaire psychotische depressie farmacologisch het beste kan worden behandeld.
In een Cochrane meta-analyse bleek dat niet aangetoond is dat behandeling met een antidepressivum plus een antipsychoticum effectiever is dan alleen een antidepressivum maar wel effectiever dan alleen een antipsychoticum. Desondanks wordt de combinatie in richtlijnen voor de behandeling van psychotische depressie vaak aanbevolen. Richtlijnen zijn dus niet altijd evidence-based.
In een gerandomiseerde dubbelblinde multicenter trial (RCT) bij patiënten met een psychotische depressie (N = 122) werden de antidepressiva imipramine en venlafaxine vergeleken met de combinatie van een antidepressivum plus een antipsychoticum (venlafaxine plus quetiapine). Op de primaire uitkomst (respons) was venlafaxine plus quetiapine effectiever dan venlafaxine met geen significante verschillen tussen venlafaxine plus quetiapine en imipramine en tussen imipramine en venlafaxine. De combinatie werd goed verdragen, maar vertoonde wel de grootste gewichttoename.
In een open studie werden de responders van de RCT nog 4 maanden gevolgd. Vervolgbehandeling met dezelfde medicatie als waarmee respons werd bereikt, bleek effectief waarbij de meeste patiënten nog verder verbeterden. Non-responders van de RCT werden nog 4 weken dubbelblind doorbehandeld in combinatie met lithium. Negen van 15 patiënten werden alsnog responder.
Conclusie: Met de gegevens uit de nieuwe RCT wordt ook in de meta-analyse het verschil tussen combinatiebehandeling en alleen een antidepressivum statistisch significant. Er is dus voor het eerst bewijs dat een psychotische depressie beter behandeld kan worden met de combinatie van een antidepressivum plus een antipsychoticum (venlafaxine plus quetiapine) dan met alleen een antidepressivum (venlafaxine).
