Asthma is a chronic disease that is characterized by inflammation, obstruction and hyperresponsiveness of the airways. Inflammatory and structural changes in the large conducting airways contribute to asthma symptoms. In the past 15 years there has been a renewed and increasing interest in the contribution of small airways, defined by an internal diameter ≤ 2 mm, to asthma. It is yet still not clear to which extent small airway involvement plays a role in asthma, and if certain subgroups of patients with asthma may specifically benefit from treatment directed towards the small airways. To further explore this, tools to measure and monitor small airway inflammation and function are needed. The studies in this thesis focussed on different small airway parameters reflecting small airway patency and inflammation and their ability to detect and monitor small airway involvement in asthma.
